NHS App

A simple and secure way to access a range of healthcare services on your smartphone via the NHS App available on both android and iOS devices. You can also access the NHS App through your web browser www.nhsapp.service.nhs.uk/login

You can use the NHS App to:

  • Demonstrate your COVID-19 vaccination status
  • Request prescriptions
  • Check symptoms and get instant advice
  • View parts of your medical records including information about medications, allergies, vaccinations, previous illnesses and test results

Please complete an online registration form if:

  • You are unable to verify your identity through the app
  • You would like to donate 'proxy access' to allow another person access to your medical records
  • You would like 'proxy access' to your child's medical records (for children under 10 years of age)

Complete the form and return to reception along with photographic identification (passport or driving licence). If you are unable to provide photographic identification the receptionist will require you to answer some questions regarding your medical history to confirm your identity.

Once your identity has been confirmed you will be provided a 'linkage key' via email with instructions on how to link your online account to your medical records.


 

 


Proxy Access

If you are managing the health of another person 'proxy access' can provide online access to:

  • view appointments
  • order medication
  • access medical records

For the other person, on their behalf.

Proxy access on behalf of children

People with parental responsibility for children have rights to access their children’s records. This is in most cases. Not all parents have parental responsibility. Proxy access for parent and guardian to a child’s record is a practice-level decision. You may also be requested to provide proof of parental responsibility if this is not already held by the practice.

Children aged 0-10

Please complete Section A (your details) and Section C (your children's details) of the online registration form and return to Surgery.

Children aged 11-15

In line with the Royal College of General Practitioners Guidance (page 4) Sidcup Medical Centre does not provide proxy access to patients aged 11-15.
Please discuss with the practice if you believe there are exceptional circumstances that would require you to retain proxy access after your child’s 11th birthday.

Proxy access on behalf of another adult

A person must be registered for online access at the practice where the patient they are acting for is registered. Proxy is the recommended alternative to sharing login details.

Adults aged 16 years and older

Adults aged 16 years and older can donate proxy access to one or more relatives, carers, or other representatives (18 years and older) to act on their behalf.
Please complete Section A (your details) and Section B (your representative, to have proxy access) of the online registration form and return to Surgery.

Proxy access on behalf of patients with a Lasting power of Attorney

If you are applying for proxy access to a patient with a Lasting power of Attorney (LPA) or, Court of Protection Order please complete online registration form Section A with the patients details, and Section B with your (the Attorney / Deputies) details. Please return the completed form to reception, along with your photo ID and a copy of the LPA, or Court Order.
